Commissioning and Start-up Quality Assurance
3.4.1 Scope
The
primary objective of the commissioning and start-up quality program is
to ensure that the technical aspects of the preliminary commissioning
and start-up plan are implemented effectively. This will ensure that
all safety related systems and equipment have met their intended
performance prior to operation of the facility. Although the plan
applies primarily to safety systems and equipment, it may be applied
to other areas when considered appropriate.
3.4.2
Responsibilities
The
commissioning Team will have the responsibility for the development
and implementation of the commissioning and start-up quality plan;
however, Quality Assurance will provide assistance in the development
of the necessary programs and procedures and effect their subsequent
monitoring for compliance.
The
commissioning and start-up activities will be performed by the
Commissioning Team supported by Licensors, Engineering, Plant
Operations Personnel and Vendor representatives.
The
process operators and maintenance personnel of the Client and
Commissioning Group will perform the commissioning activities; the
supervisors and mangers will verify the conformance of the work with
established procedures: and Project Quality Assurance will audit the
complete program to ensure that both performance and verification are
effective.
The
training of personnel involved in this section of the quality plan
will be conducted in timely manner to ensure a through understanding
of the issue by plant management, plant supervision, maintenance
personnel and process operators of operating manuals, maintenance
manuals, start-up/shutdown and emergency procedures, unit drawings and
unit/plant models.
The
commissioning and start-up quality program will be documented and
implemented as follows:

Commissioning activities will be carries out in accordance with
written procedures. The peron/group with authority to execute the work
will be identified.
Systems and equipment under commissioning control will be clearly
identified by permits, tags, etc. with particular attention to the
terminal points.

Instructions, Procedures, Drawings
Instructions, procedures and drawings will be developed, as
appropriate, to describe the proposed commissioning activities.
Prior
to the commencement of commissioning, the person/group with authority
will clarify the correct sequence of activities, the prerequisites,
the resource materials and the test objectives.
The
results of the commissioning tests will be recorded to provide
evidence that the design intent has been achieved. These records will
from a part of the permanent facility records.

Document Control
The review, approval, issuance,
distribution and revision of documents will be handled by operations .

Receiving , Handling and Storage
Procedures will be developed to control receipt, handling and storage
of materials, equipment and test instruments, particularly with
respect to spares required for the activities.
All
equipment will be visually examined for transit damage to establish
conformity to the documentation arc: inspected to ensure specified
packaging is correct.
Equipment found to be 'nonconforming' will be rejected or will undergo
corrective action to upgrade it to an acceptable level.
Handling procedures will be developed especially for materials and
equipment which may be damaged if handled incorrectly.
Storage procedures will be developed such that items susceptible to
environmental deterioration will be stored under appropriate
conditions.

Calibration of Measuring and Testing Equipment
Procedures will be developed to ensure that all tools, gauges and
instruments are of the proper range, type and accuracy.
The
equipment will be adjusted, calibrated and maintained against
nationally recognized standards to ensure accuracy. Records will be
maintained and the equipment will be marked to indicate its
calibration status.

Records
A
record system will be developed to collect and maintain design,
manufacturing and construction documents required to commission, the
commissioning records and the audit results.
